Do you accept insurance?

All visits are considered out-to-network (OON) and will fall under your unique OON benefits of your insurance. Depending your coverage you may receive a significant portion of your initial payment back directly from your insurance provider. You will be provided a superbill after each visit to submit your claim for reimbursement.

HSA/FSA is accepted.

Why don’t you accept insurance?

Sadly, insurance often dictates what treatment a physical therapist can perform and for how long. It also can be more expensive for the patient than cash-based rates. Accepting insurance often leads to more frustrating results for both client and clinician. Dr. Eberle believes cost transparency is vital to patient-clinician trust relationship.

What are your rates then?

Initial evaluation & Intake (1 hr): $180

Follow ups (50 minutes): $150

Dry needling add-on : +$20 to follow up

How many sessions should I expect? How often?

One of the best parts of working with an out of network provider is that you and the provider have full autonomy as to how often you have sessions and can prioritize whatever is most important to you in that session. Often with a new injury or recovery a client will be recommended weekly visits. Many of our patients after the acute injury has resolved will follow up 1x/month to help continue to support their goals (or create new ones!). Check ins with a physical therapist can be very helpful for injury prevention in whatever movements you love most.

How about Medicare clients?

Dr. Eberle is working on this! At this time, Dr. Eberle is not able to accept clients with Medicare until she has been credentialed. After she is verified a provider, she will be in network with Medicare. Check back for updates. Sorry for the inconvenience at this time.

Where do I park for my appointment?

Parking Restrictions:

4102 Falls Road is located on the West side of the street, near the corner of 41st and Falls Road.

Domino’s Pizza is directly opposite, on the East side of the street.
Before 9:00 AM - NO PARKING on West side of street (same side as 4102 Falls Road)
After 4:00 PM - NO PARKING on East side of street (Domino’s pizza side)

Handicapped access:
If you need assistance with stairs, arrange to park at the rear of the building. Erin will assist you in using the motorized incline stair lift which provides direct access to the office.

What do I wear to my appointment?

Comfortable attire is encouraged.

What should I expect when I arrive?

Please ring the doorbell when you arrive and enjoy the porch. I will let you in when I have finished with the previous client.
